Please Read Before Completing the Interest Form
Clayton County Coordinated Entry is a process that assesses your housing situation and tries to connect or refer you to an appropriate housing program or community resource when one is available. Coordinated Entry itself is not a housing program. The Interest Form is not an application for housing or rental assistance and does not mean you are approved for a program. It is the first step used to understand your situation and determine appropriate next steps and resources.
Rental Assistance / Homeless Prevention
Hearts does not currently operate a Homeless Prevention/Rental Assistance program. You may be placed on the Pre-Waiting List for possible referral if an appropriate program begins accepting applications. Please continue checking the Resource Page at ClaytonCountyCOE.org for other agencies and rental-assistance opportunities.
If You Are Homeless
After you submit a completed Interest Form, Hearts staff will attempt to call you within 48 business hours to complete your Coordinated Entry Assessment.
If we are unable to reach you by phone, we will email and/or text you information directing you to ClaytonCountyCOE.org to schedule your assessment. Please make sure the phone number and email address you provide are correct and check your Spam/Junk folder for messages from Hearts.
Your assessment must be completed or scheduled within one week of submitting your Interest Form. If it is not completed or scheduled within that time, your Coordinated Entry case will be closed.
If you still need assistance after your case is closed, you must request reactivation of your existing case by emailing coe@heartstonourishhope.org. Do not complete another Interest Form. After your assessment, you may be placed on the Pre-Waiting List if appropriate. For Rapid Rehousing, the current estimated wait is approximately 6–8 weeks or longer on the Pre-Waiting List before you may be contacted to complete a Pre-Application and begin the process of determining eligibility.
Emergency Shelter / Street Outreach
Emergency Shelter and Street Outreach resources or referrals may be available based on your situation and current availability. Emergency Shelter placement is not guaranteed.
Support While You Wait
Hearts offers stabilization and support programs on:
Wednesdays — Programs begin at 10:00 AM
1st and 3rd Saturdays — Programs begin at 10:00 AM
Please arrive by 10:00 AM. These are scheduled programs and are not open drop-in hours where you may arrive at any time during the program period.
Participation in these programs does not guarantee housing, rental assistance, financial assistance, or movement ahead on the Pre-Waiting List.

At Clayton County Coordinated Entry, we are committed to ending homelessness in our community. We offer a range of services to support homeless families, including emergency housing, food assistance, job training, and healthcare services. Our team works closely with each family to tailor our services to meet their unique needs and help them get back on their feet. Our goal is to make a positive impact in the lives of homeless families in Clayton County.
At Clayton County Coordinated Entry, our latest project is focused on providing emergency housing to homeless families in our community. We are working with local partners to identify families in need and provide them with safe, stable housing as quickly as possible. Our team is also providing ongoing support to these families to help them overcome the challenges that led to their homelessness and achieve long-term stability.
